Lip Flip vs Lip Filler: Which Lip Treatment Is Right for You?

Lip Flip or Lip Filler?

Two very different treatments, one shared goal. Here is how to choose well.

Lip flip and lip filler are the two treatments patients most often confuse, and they work in completely different ways. At Dr Harry Clinic, led by GMC-registered aesthetic doctor Dr Harry James in Chiswick and Covent Garden, London, choosing between them starts with your anatomy and your goal, not with a trend. This guide explains both, side by side.

Quick Answer: a lip flip relaxes the muscle around the upper lip with a small dose of anti-wrinkle injection so the lip rolls subtly outward, showing more of the lip you already have. Lip filler adds hyaluronic acid gel to create volume, structure and definition. A flip changes position, filler changes shape. They can also work together.

What does a lip flip actually do?

A lip flip uses a few precise, very small doses of anti-wrinkle injection placed along the upper border of the orbicularis oris, the ring of muscle that circles the mouth. Relaxing those fibres lets the upper lip roll gently outward, so more of the pink lip shows when your face is at rest and when you smile.

Nothing is added to the lip itself. There is no volume change, no extra weight and very little downtime. The effect is deliberately subtle: patients usually describe it as looking like themselves on a good day rather than looking done. Because the treated muscle moves constantly when you talk, eat and smile, a lip flip wears off faster than anti-wrinkle treatment elsewhere, typically within around two to three months.

A lip flip is also one of the tools we use for a gummy smile and for fine vertical lines around the mouth, where relaxing overactive muscle does most of the work. You can read more about the treatment itself on our lip flip page.

What does lip filler actually do?

Lip filler is an injectable gel made of hyaluronic acid, a molecule your skin already produces, placed with fine needles or a cannula to build volume, restore symmetry, define the border or support the corners of the mouth. Our lip filler page covers the full treatment journey. Unlike a flip, filler physically changes the shape and structure of the lips.

Modern lip work is about proportion rather than size. A well-judged treatment respects the natural ratio between upper and lower lip, the shape of your cupid’s bow and how your lips sit against the rest of your face. Results typically last around six to twelve months in the lips, because constant movement gradually breaks the gel down, and hyaluronic acid filler is reversible if ever needed.

Which one fits your goal?

Choose by what bothers you, not by what is trending. If your upper lip is naturally reasonable in size but tucks in and vanishes when you smile, a flip alone can be enough. If the lip lacks volume at rest, filler is the only one of the two that will change that, because a flip cannot create tissue that is not there.

A simple way to think about it: a lip flip reveals, filler builds. Gummy smile or hidden upper lip, think flip. Volume, border definition or asymmetry, think filler. Fine smoker’s lines respond best to muscle relaxation, sometimes with a light touch of filler or a skin booster alongside.

Can you combine a lip flip with filler?

Yes, and the combination is often where the most natural results live. Filler provides the structure and definition, while a flip adds a subtle outward roll that keeps the result soft rather than heavy. The two are usually staged rather than done in one sitting, so each can be judged on its own merits.

For lips that need surface quality rather than shape, hydration-focused skin boosters such as the Revive Lip Booster are a third option worth knowing about, improving texture and softness without adding volume. A consultation is where we map which combination, if any, actually serves your face.

Technique and safety

The lips and the area around the mouth are unforgiving of imprecise work. The muscle doses in a lip flip are tiny and placement matters: too much, or in the wrong spot, can temporarily affect how the mouth moves. Filler placed without respect for lip anatomy can look heavy or migrate above the lip line over time.

Dr Harry James is a GMC-registered doctor and ACE Group World faculty member, which means complication prevention and management are built into how every treatment here is planned. If you have existing filler from elsewhere that has migrated or sits unevenly, ultrasound-guided dissolving can reset the canvas before any new work is considered.

Frequently asked questions

How long does a lip flip last?

Typically around two to three months, which is shorter than anti-wrinkle treatment in areas like the forehead. The muscle around the mouth works constantly, so it recovers its movement sooner. Many patients time top-ups two or three times a year.

Does a lip flip make your lips bigger?

No. It does not add any volume. It relaxes the muscle at the upper lip border so the lip rolls slightly outward, showing more of the lip you already have. If you want genuinely fuller lips, filler is the treatment that does that.

Which is better for smoker’s lines?

Fine vertical lines around the mouth are usually driven by muscle activity, so small relaxant doses are the first-line approach, occasionally supported by a light skin booster or filler for etched-in lines. The right mix depends on how deep the lines are, which is assessed in consultation.

Is lip filler reversible?

Yes. Hyaluronic acid filler can be dissolved with an enzyme called hyaluronidase if the result is not right or old filler has migrated. At Dr Harry Clinic this is done with ultrasound guidance for precision.

Can I have a lip flip and filler at the same appointment?

They are usually staged a couple of weeks apart so each result can be assessed properly before the next step. Your treatment plan, including sequencing, is agreed at consultation in Chiswick or Covent Garden.

Dr Harry is absolutely amazing, I can’t recommend him enough. I had my usual skin maintenance treatments yesterday, and for the first time I decided to try a lip booster. Harry was so informative, knowledgeable, and reassuring throughout, and he made me feel completely at ease. I was originally offered standard lip filler, but as I’m in my 60s I wanted something more subtle and natural-looking, the lip booster felt like the perfect choice. I left the clinic with only a little swelling (nothing too bad at all) and my lips already look healthier, more youthful, and beautifully natural. I’m genuinely thrilled with the result and can’t thank him enough. Five-star-plus treatment every time. His prices are very reasonable, and his manner with patients is kind, professional, and wonderful. I’m already looking forward to my next appointment, even if I have to wait a few months because the results last so well. Thank you Dr. Harry ❤️

I visited Dr Harry after having my lips done by a few, other practitioners and my experience was positively incomparable. Harry was extremely thorough with his questioning, support and actual practice, I felt reassured and at ease, alike did my Mum, at the hands of Harry. I had 0.6ml of lip filler and was extremely impressed that he reevaluated my lips afterwards, adding in the remaining filler into specific areas. I did not have 1 single bruise, despite coming up black and blue in the past, and my lips are the most symmetrical they have ever looked. 1 million percent the best investment I have made to my face and I would never recommend going to anybody else! I visited the Chiswick clinic and it was beautiful🫶🏼 thank you Harry

I went to Dr Harry Clinic after both a recommendation and doing a lot of research, as this was my first anti-wrinkle injection treatment and choosing someone I truly trusted was really important to me. From the moment I arrived, Dr. Harry immediately made me feel at ease. He was so lovely, professional, and took the time to explain everything in detail. Most importantly, he really listened to what I wanted - especially my preference for a very natural look. Two weeks later, I had my complimentary follow-up appointment and I was already so happy with the results. At the follow-up, Harry was an absolute perfectionist, carefully assessing everything to ensure it was as balanced and natural as possible, with a small top-up where needed. Overall, my experience exceeded all expectations. If it’s your first time having treatment, or you’re looking for a new injector, I genuinely cannot recommend Dr Harry Clinic enough. One of the best decisions I’ve made.
